Troubleshooting sound output device issue

I have a issue where sounds dont work and it seems quite random. If i open up the mixer 'after a good boot' i can see items like below in the mixer when i have opened up my apps.

System Sounds 100% (0db)
'Firefox USB Audio Analog Stereo'
'Spotify USB Audio Analog Stereo'

But sometimes when the machine starts you can just see in the mixer that there is usually only one item tops in the mixer be it spotify or Firefox it usually only works if i redirect to HDMI TV-screen that i use. It basically behaves like sound has been reserved by one app only and nothing else can use the device. Normally i can play spotify (flatpak) and firefox on top of each other if i need to. Now also it never goes to HDMI automatically but once i troubleshoot it away from the standard USB Audio Analog Stereo that is the only way to get sounds to work at all. If i put something to play i can see the pulsating bar move under the device item meaning there is some type of audio signal being processed but it refuses to actually output sound to the speakers.

Got any advice on what to look for? I find this a bit tough to understand as it does work but then it doesn't and it's random. I dont move/touch any wires or anything so i doubt it is connection failing issue e.g. bad cable+detection failing somehow.
